Services Description

Anchor Longevity is a clinician-led practice specializing in peptide therapy and longevity optimization. Services include clinical evaluations, laboratory analysis, personalized treatment protocols, and ongoing clinical monitoring, provided by qualified, board certified providers.

Eligibility

• Age: You must be 18 or older.
• Geographic restrictions: Services available only where our clinician is licensed to practice.
• Medical appropriateness: Acceptance is at the sole discretion of our clinical team following evaluation.

Membership Terms and Billing

Monthly Membership

• $25 per month, billed monthly from your enrollment date.
• Includes personalized protocol design, monthly clinician check-ins, quarterly protocol reviews, secure messaging, and 20% off peptide orders.

Annual Membership

• $800 per year (saves $100 vs. monthly).

Clinical Evaluation

• $25 for a 20-minute session, credited toward first month's membership if you join.
• Non-refundable once conducted.

Peptide Purchases

• Purchased separately at member pricing (20% discount). Requires valid prescription.

Lab Billing

• Billed separately through our partner lab. Typical range $150-300 depending on panel. Not included in membership.

Program Description

We send SMS messages for appointment confirmations, appointment reminders, product availability updates, promotional offers, and follow-up communications related to your care.

Message Frequency

Message frequency varies with your interactions and any scheduled appointments or promotions (typically 2–4 messages per month).

Rates

Message and data rates may apply. Check with your mobile carrier for details.

How to Opt In

Submit your mobile phone number through our website consultation booking, contact form, or chat widget and indicate your consent to receive SMS messages.

How to Opt Out

Reply STOP at any time to unsubscribe. You will receive one final confirmation and no further messages will be sent.

Cancellation and Refund Policy

• Monthly membership may be cancelled at any time; no cancellation fees.
• Annual memberships may be cancelled with prorated refund at our discretion.
• We assist with safe off-ramping from active protocols upon cancellation.
• Consultation fees are non-refundable once services have been rendered.

Acceptable Use Policy

• Provide accurate information during intake or consultations
• Do not share, resell, or redistribute prescribed medications
• Use services for your own personal healthcare only
• No unauthorized access to our systems or records

Limitation of Liability

Anchor Longevity shall not be liable for indirect, incidental, or consequential damages. Clinical outcomes are individual and cannot be guaranteed. Liability is limited to fees paid for the specific services giving rise to the claim.

Medical Disclaimer Reference

Use of our services is also subject to our Medical Disclaimer, incorporated by reference.

Clinician-Patient Relationship

Submitting a consultation request does not establish a clinician-patient relationship. That relationship is formed only after formal acceptance, completion of intake, and a direct consultation with our clinician.

Intellectual Property

All content on this website is the property of Anchor Longevity and protected by intellectual property laws.

Dispute Resolution

Disputes are resolved through good-faith negotiation, then binding arbitration under AAA rules conducted in Oklahoma.

Governing Law

These Terms are governed by the laws of the State of Oklahoma.

Severability, Entire Agreement, Modifications

If any provision is unenforceable, the remainder remains in effect. These Terms, together with our Privacy Policy and Medical Disclaimer, are the entire agreement. We may modify these Terms; continued use constitutes acceptance.
