Services Description

Anchor Longevity is a clinician-led practice specializing in peptide therapy and longevity optimization. Our services include clinical evaluations, laboratory analysis, personalized treatment protocols, and ongoing clinical monitoring. All services are provided by qualified, board certified providers.

Eligibility

• Age: You must be 18 years of age or older to use our services.
• Geographic restrictions: Services are available only in states where our clinician is licensed to practice. Licensing status is verified during the intake process.
• Medical appropriateness: Not all individuals are candidates for peptide therapy. Acceptance into our program is at the sole discretion of our clinical team following evaluation.

Membership Terms and Billing

Monthly Membership

• Membership is $25 per month, billed on a monthly cycle from your enrollment date.
• Membership includes: personalized protocol design, monthly clinician check-ins, quarterly protocol reviews, secure messaging access, and 20% off all peptide orders.

Annual Membership

• Annual membership is $800 per year (a savings of $100 vs. monthly billing).
• Annual memberships include the same benefits as monthly memberships.

Clinical Evaluation

• The Clinical Evaluation costs $25 for a 20-minute session.
• The consultation fee is credited toward your first month's membership if you choose to join.
• The consultation fee is non-refundable once the consultation has been conducted.

Peptide Purchases

• Peptides are purchased separately from membership at member pricing (20% discount).
• All peptides require a valid prescription following clinical evaluation.

Lab Billing

• Laboratory testing is billed separately through our partner lab.
• Typical costs range from $150-300 depending on the panel ordered.
• Lab costs are not included in membership fees.

SMS Messaging Terms

By opting in to receive text messages from Anchor Longevity, you agree to these SMS Messaging Terms.

Program Description

Anchor Longevity sends SMS messages for appointment confirmations, appointment reminders, product availability updates, promotional offers, and follow-up communications related to your care.

Message Frequency

Message frequency may vary based on your interactions with us and any scheduled appointments or promotions (typically 2–4 messages per month).

Rates

Message and data rates may apply. Check with your mobile carrier for details.

How to Opt In

You may opt in by submitting your mobile phone number through our website consultation booking, contact form, or chat widget and indicating your consent to receive SMS messages.

How to Opt Out

Reply STOP at any time to unsubscribe from all SMS messages. You will receive a final confirmation and no further messages will be sent.

Privacy

Cancellation and Refund Policy

• Monthly membership may be cancelled at any time. No cancellation fees apply.
• Annual memberships may be cancelled with a prorated refund for unused months at our discretion.
• We will assist with safe off-ramping from any active protocols upon cancellation.
• Consultation fees are non-refundable once services have been rendered.

Acceptable Use Policy

You agree to use our website and services only for lawful purposes and in accordance with these Terms. You agree not to:

• Provide false or misleading information during intake or consultations
• Share, resell, or redistribute prescribed medications to others
• Use our services for purposes other than your own personal healthcare
• Attempt to gain unauthorized access to our systems or patient records

Limitation of Liability

Anchor Longevity and its clinical staff shall not be liable for any indirect, incidental, or consequential damages arising from the use of this website or our clinical services. Clinical outcomes are individual and cannot be guaranteed. Our liability is limited to the fees paid for the specific services giving rise to the claim.

Medical Disclaimer Reference

Use of our services is also subject to our Medical Disclaimer, which is incorporated into these Terms by reference.

Clinician-Patient Relationship

Submitting a consultation request does not establish a clinician-patient relationship. Such a relationship is formed only after formal acceptance into our clinical program, completion of the intake process, and a direct consultation with Courtney Hammond, APRN-CNP.

Intellectual Property

All content on this website — including text, graphics, logos, images, and design — is the property of Anchor Longevity and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written permission.

Dispute Resolution

Any disputes arising from these Terms or our services will be resolved through good-faith negotiation. If negotiation fails, disputes will be submitted to binding arbitration in accordance with the rules of the American Arbitration Association, conducted in Oklahoma.
